Return Policy

We want you to be completely satisfied with your purchase. If you are not satisfied with your merchandise, we will accept returns within 15 days of receipt for a refund.

ALL RETURNS REQUIRE A RETURN AUTHORIZATION NUMBER (RMA). Please contact us at returns@form-baby.com with your name, order ID number, item name and reason for return. You will receive a Return Authorization Number (RMA) via email. This number must be clearly marked on the outside of the package when shipped back to FORMbaby. RMA numbers are valid for 10 calendar days from receiving, so promptly ship back the item.

Return shipping costs are the sole responsibility of the customer. If any item is returned to us damaged, we will be unable to issue a refund for the item. Please insure your package when shipping. Once we receive your package, we will refund your credit card. We will notify you via email once your return has been processed. To order a different item, place the new order separately on our website.
